What do I need to get a Deed Poll?
If you wish to change your legal name, you need to complete a Deed Poll. You state that you formally renounce your previous name and will only be known by your new name from the date specified.
Anyone can apply for a Deed Poll. You can use one in order to change your name on official documents such as passports, driving licences and any other legal identification.
You can create a Deed Poll for a Change of Name yourself, there are many online templates available and anyone can witness this for you. This will need separate solicitor certification that is independent of the Deed Poll itself and only for the apostille.
